(NASDAQ: TZIX) today announced the limited availability release of CareAdvance Medicare, for proactive care management of Medicare members. Designed to address the unique needs of senior populations and Medicare Advantage (MA) plans, CareAdvance Medicare:
- Automates case and disease management activities
- Provides case management support specific to Medicare beneficiaries
- Features a new Medicare Health Risk Assessment for seniors
- Enables MA plans to align with new guidelines of Special Needs Programs (SNPs)

CareAdvance Medicare will be sold as an optional component to TriZetto's Clinical CareAdvance (CCA) application, the clinician-facing feature of the CareAdvance Enterprise (CAE) solution. Leveraging CAE's secure, Web-based platform, CareAdvance Medicare is the first solution of its kind to successfully automate the full spectrum of care management for both commercial and Medicare members.
